ATSICHS is currently seekingAboriginal Health Workers for its Northgate, Woolloongabbaand Logan clinics.
The successful applicants will perform the following duties:

To work as part of a professional clinical services team to provide the highest quality and culturally appropriate services to the Indigenous community

Implement and manage with the team an effective Patient Recall system, Health Check and Care Plan in line with ATSICHS policies and procedures

Undertake community outreach, hospital and home visits
-
Provide health screening, health promotion and education activities.

Advocate for health issues on behalf of Aboriginal and Torres Strait Islander people
-
Facilitate and follow up referrals for clients
-
Record and maintain statistical data for program analysis, monitoring and other reporting purposes as required.
Essential criteria
-
Ability to sensitively and effectively communicate with Aboriginal and Torres Strait Islander people;

Current Drivers Licence;
-
Current Suitability Check or ability to obtain one(Blue Card)

Possession of a minimum Certificate III or IV in Indigenous Primary Health Care
Desirable
-
Two (2) years experience as part of a multi disciplinary team in the delivery of Primary Health Care Services.
